Ĭare bundles combine evidence-based interventions, usually three to five components, to yield a significantly better outcome than when individually implemented. The Braden scale is highly effective in assessing HAPU risk among patients in medical, surgical, and critical care settings, and is more accurate than the clinical judgement of nurses. The Braden scale for predicting pressure sore risk, more commonly called the Braden scale, is the most widely used tool in hospitals to identify patients at high risk for HAPUs.
